Who covers it?
A synthetic scenario. The rule names and the refusal are the system's own; SITE_ELIGIBILITY and COMPETENCY are two of eight hard constraints.
Every change is written down. Alter one entry and the chain stops matching.
Every plan, override, refusal and approval goes to an append-only, hash-chained log with who acted and why.
Start with phase zero. It retrains on your own history and checks the forecast against the ratio you use today. If it does not beat it, the engagement stops there, and that finding is yours to keep.
Phase zero runs on an export of past sessions: counts by appointment type, complexity flags and realized technician minutes. The contract carries no patient name, date of birth, diagnosis or note. Whether the combination is protected health information in a low-volume clinic is settled with your privacy office before any file moves. Pricing for the pilot is set per engagement.
